How to Apply for an Internship Liability Insurance Certificate

For 2026 students in the Faculty of Science and Technology at Saitama Hatoyama Campus, the internship destination must be reported before submitting the certificate application.

Application procedure

First, report your internship destination using the university website’s “Internship Destination Information” form. Then, separately enter the required information in the “Internship Liability Insurance Certificate Application for Saitama Hatoyama Campus” form to apply for your internship liability-insurance enrollment certificate.
