Leave of Absence Request Form and Required Signatures

For the 2026 academic year, students in the Faculty of System Design and Technology at Tokyo Senju Campus must complete the Leave of Absence Request and obtain signatures and seals from both the student and the guarantor.

Completing the Leave of Absence Request

Obtain the Leave of Absence Request from the Student Welfare Desk and complete all required fields. The form must include the signatures and seals of both the student and the guarantor.
