How to Complete the Withdrawal Request Form

This page explains where to obtain the Withdrawal Request Form and whose signatures and seals are required.

Obtain and complete the form

For the 2025 academic year, students at Tokyo Senju Campus in the Faculty of System Design and Technology must obtain the Withdrawal Request Form from the Student Welfare Desk and complete all required information. The student and guarantor must sign the form jointly and affix their seals.
