How to Submit Group Travel Forms

Students using group travel must complete two forms and submit them to the Student Support Center.

Required forms and submission

For group travel, complete the required information on both the Off-Campus Activities Notification and the Group Travel Application Form. Submit the completed forms to the Student Support Center, Student Welfare Section. The Group Travel Application Form is available at stations and travel agencies.
