Submitting an Absence Notification

If you must miss class for an unavoidable reason, such as bereavement, illness or injury, or extracurricular activities, obtain the absence notification form from the Administrative Office's Academic Affairs desk at Tokyo Senju Campus or from the university website. Complete the required information, attach documents that verify the reason for your absence, such as a medical certificate, and submit the form to the instructor in charge of the class. Explain the circumstances when you submit it.
