Documents Required for a Leave-of-Absence Application

For the 2025 academic year, students in the Faculty of Future Science at Tokyo Senju Campus must submit the documents listed below when applying for a leave of absence.

Required documents

A leave-of-absence application requires the application form, a medical certificate or other supporting document where applicable, and a receipt for payment of the enrollment fee.
