For the 2026 academic year, students at the Tokyo Senju Campus in the School of Future Science need an application form and a medical certificate or similar document for reinstatement.
Required Documents and Office
A reinstatement application requires an application form and a medical certificate or similar document. The responsible office is the Student Support Center, Student Welfare Affairs, on the third floor of Building 2.
