Documents Required for a Change-of-Surname Notification

For the 2025 change-of-surname notification procedure at the Tokyo Senju Campus Faculty of Engineering, the required attachments depend on whether the notification is for a student or a guarantor.

Required Attachments

For a student's or guarantor's change-of-surname notification, attach a document showing the change, such as an extract from the family register. If the notification is for a student, also attach the student's ID card.
