For students in the Faculty of Science and Engineering at Saitama Hatoyama Campus, obtain it from the Faculty of Science and Engineering Administrative Office, Student Affairs/Welfare. For students in the Faculty of Engineering, Division II, at Tokyo Senju Campus, the form is available from the Student Support Center (Student Welfare).
