How to Reapply for a Student ID

This page explains where to obtain and submit the student ID reissuance application form, plus the application cost for the applicable academic year and faculty.

Application procedure

For the 2025 academic year at Senju Campus in the Faculty of System Design and Technology, print the student ID reissuance application form using the certificate-issuing machine on the third floor of Building 2. Fill in the required information and submit the completed form to the Student Support Center (Student Welfare). For the 2026 academic year at Senju Campus in the Faculty of Engineering, obtain the form from the certificate-issuing machine on the third floor of Building 2, complete the required information, and submit it to the Student Support Center (Student Welfare).

Application cost

For the 2025 academic year at Senju Campus in the Faculty of System Design and Technology, the student ID reissuance application form costs 2,000 yen. A separate system fee is also charged.
