How to Submit an Absence Notification

Learn where to obtain the absence notification form, what supporting documents to attach, where to submit it, and who assesses it, according to your campus, faculty, and academic year.

Tokyo Senju Campus: 2026

For students in the Faculty of Engineering or the School of System Design and Technology who are unavoidably absent, for example because of bereavement, illness or injury, or extracurricular activities, obtain the absence notification form from the Tokyo Senju Campus Administration Office (Academic Affairs) or the university website. Complete the form, attach documentation verifying the reason for the absence, such as a medical certificate, and submit it to the instructor while explaining the reason for your absence.

Tokyo Senju Campus: 2025

For students in the Faculty of Engineering, if you are unavoidably absent because of bereavement, illness or injury, extracurricular activities, or a similar reason, obtain the absence notification form from the Tokyo Senju Campus Academic Affairs counter or the university website and complete the required information. Attach documents that can verify the reason for your absence, such as a medical certificate, submit the form to the course instructor, and explain the reason for your absence.

Saitama Hatoyama Campus: 2026

For students in the Faculty of Science and Engineering, if you miss a class for an unavoidable reason such as illness or injury, obtain the absence notification form from the Academic Affairs counter and complete the required information. Submit the form with documents, such as a medical certificate, that verify both the absence and the period of absence. The university, including the course instructor, makes the decision regarding the absence notification.
