Submitting Documentation for an Absence

For the 2025 academic year at Saitama Hatoyama Campus in the Faculty of Science and Technology, learn how to submit an absence-notice form and when the student section must be completed and signed or sealed.

How to submit an absence notice

If you miss class for an unavoidable reason, such as illness or injury, obtain an absence-notice form from the Academic Affairs office. Complete the required fields and submit the form with documentation proving both the absence and the period for which the absence was necessary. For example, a medical certificate stating the treatment period with dates can be used.

Student section requirement for shorter absences

For an absence lasting six or fewer consecutive days, the student’s entry and seal or signature in the student section of the absence-notice form are required.
