Leave of Absence: Eligibility, Application, and Required Documents

This page explains when a leave of absence requires permission, how to obtain and complete the application, and which documents and submission offices apply to each listed faculty, campus, and academic year.

When permission is required

For the 2025 academic year in the Faculty of System Design, if illness, injury, or another reason prevents you from attending classes for three months or longer, you must submit a leave-of-absence application and obtain permission.

Obtaining and completing the application

The leave-of-absence application is obtained from the Student Support Center's Student Welfare Desk. Complete the required information on the designated form. The student and guarantor must sign jointly and affix their seals. For the 2026 academic year in the Faculty of Engineering, the Faculty of Engineering (Evening Division), and the 2025 academic year in the Faculty of Future Science, leave-of-absence procedures are handled by the Student Welfare Desk.

Required documents and submission offices

The leave-of-absence application requires the designated application form, supporting documents such as a medical certificate where applicable, and proof of payment of the enrollment fee. For the 2026 academic year in the Faculty of Engineering, the application is handled by the Student Support Center's Student Welfare Desk on the third floor of Building 2. For the 2026 academic year in the Faculty of Engineering (Evening Division) and the Faculty of Future Science, and the 2025 academic year in the Faculty of System Design, the listed required documents are the application form, supporting documents such as a medical certificate, and the enrollment-fee payment receipt. For the 2026 academic year in the Faculty of System Design, submit the application to the same Student Welfare Desk on the third floor of Building 2, together with the application form, supporting documents such as a medical certificate, and the enrollment-fee payment receipt. For the 2025 academic year at the Saitama Hatoyama Campus in the Faculty of Science and Technology, submit the application to the Student Welfare Desk with the application form, supporting documents such as a medical certificate, and a copy of the student portion of the enrollment-fee payment form.
