Eligibility for a University-Issued Health Examination Certificate

The requirements for receiving a health examination certificate differ by academic year, campus, and faculty. The certificate generally requires completion of the applicable health examinations.

2026: Tokyo Senju Campus

For students in the Faculty of System Design and Technology and the Faculty of Future Science, a health examination certificate cannot be issued if the student has not taken the regular health examination or has not completed a required re-examination. For students in the Faculty of Future Science, the certificate is issued through the certificate issuance service.

2025: Hatoyama Campus

For students in the Faculty of Science and Technology, a health examination certificate can be issued only during the relevant academic year and only to students who took that year's on-campus regular health examination. If a student instead received a health examination outside the university and submitted the certificate, the university cannot issue a health examination certificate. This does not apply to examinations conducted by the university physician.
