Reporting Changes to Personal Information

Address and contact information

If your current address or contact information changes, submit the change notification form immediately. Contact information includes a telephone number or other number that can normally be used to reach you.

For the 2025 academic year at the Saitama Hatoyama Campus, Faculty of Science and Technology, submit the form to the Student Welfare Desk. Your student ID card is required.

For the 2026 academic year at the Tokyo Senju Campus, Faculty of Future Science or Faculty of Engineering, submit the form to the Center Head and Administrative Director.

For the 2026 academic year at the Tokyo Senju Campus, Faculty of Future Science, and at the Saitama Hatoyama Campus, Faculty of Science and Technology, an address or residence-address change must be reported to the Student Support Center, Student Welfare Desk, using the change notification form available at the desk. Changes to your address or contact information must be reported immediately to the Center Head and Administrative Director.

Changes involving your guarantor

If your guarantor changes, submit the change notification form to the Student Welfare Desk. Your student ID card is required, as well as the signatures and seals of both the new and former guarantors.

These requirements are stated for the 2025 academic year at the Saitama Hatoyama Campus, Faculty of Science and Technology, and for the 2026 academic year at the same campus and faculty.

Other personal information changes

If any personal matter listed in the applicable table changes, submit the change notification form.

For the 2025 academic year at the Saitama Hatoyama Campus, Faculty of Science and Technology, a change to your or your guarantor's surname or nationality must be reported to the Student Welfare Desk. Submit documentation showing the change, such as an extract of family-register information, together with your student ID card.
