Submitting a Student or Guarantor Name-Change Notice

Find the required documents and submission location for a student or guarantor name-change notice in the 2026 academic year.

Faculty of Engineering, Division II

For students at Tokyo Senju Campus in the Faculty of Engineering, Division II, submit the student or guarantor name-change notice to the Student Support Center, Student Welfare Section, on the third floor of Building 2. Include a document showing the change, such as an extract of the family register. If the name change is for the student, also submit the student ID card.
