How to Submit an Absence Notification

Learn how to obtain and complete an absence notification, what supporting documents to attach, where to submit it, and what additional submission is required for absences of one week or longer.

Obtain and complete the form

For unavoidable absences such as bereavement, illness or injury, or extracurricular activities, obtain the absence notification form from the Academic Affairs desk at Tokyo Senju Campus or from the university website. Complete all required fields. This procedure is specified for the 2025 academic year in the Faculty of Future Science and for the 2025 and 2026 academic years in the Faculty of System Design and Technology.

Submit the form with supporting documents

Attach supporting evidence, such as a doctor's certificate or a statement explaining the reason for the absence, to the completed absence notification. Submit it to the instructor responsible for each class and explain the absence when required. This submission procedure applies to the specified 2025 and 2026 academic-year scopes for the Faculty of Engineering, the Faculty of Engineering Second Division, the Faculty of System Design and Technology, and the Faculty of Future Science at Tokyo Senju Campus.

Additional submission for absences of one week or longer

When the absence lasts one week or longer, submit the absence notification to the instructor responsible for each class and also to the Academic Affairs desk of the Tokyo Senju Campus Administrative Office. This additional submission is specified for the 2025 and 2026 academic years in the Faculty of System Design and Technology, for the 2025 academic year in the Faculty of Engineering Second Division, and for the 2026 academic year in the Faculty of Engineering Second Division.

Procedure at Saitama Hatoyama Campus

For the 2025 academic year in the Faculty of Science and Technology at Saitama Hatoyama Campus, the absence notification is handled by the Academic Affairs desk. The relevant guidance is listed under Chapter 4, Course Registration Guide – 2. Classes – 7. Class Absences, on page 156.
