Receiving Forms for Returning from Leave

This page explains when the return-from-leave instructions and application form are mailed, and where to submit the required documents.

Instructions and Application Form

For the Faculty of Science and Engineering at the Saitama Hatoyama Campus in academic year 2025, the Student Affairs office mails the procedure instructions and the return-from-leave application form to the student when the leave period ends. For the Faculty of Engineering, Second Division, at the Tokyo Senju Campus, the Student Affairs office mails these documents to the student before the leave period ends in academic years 2025 and 2026.

Required Documents

For the Faculty of Engineering, Second Division, at the Tokyo Senju Campus in academic year 2025, submit the required documents to the Student Affairs office counter. If your leave was due to illness or injury, attach a doctor's medical certificate.
