Documents to Take to the Practicum School

For the 2026 Education Practicum at the Saitama Hatoyama Campus in the Faculty of Science and Technology, take the following documents to the practicum school:

- Education Practicum Diary: the faculty-designated form, which students must purchase individually at the campus shop. - Attendance Sheet Form: provided by the academic affairs office. - Education Practicum Evaluation Form: provided by the academic affairs office. - Education Practicum Completion Certificate Form: provided by the academic affairs office.
