Procedures for Returning from Leave of Absence

This page explains the advisor meeting, required documents, medical certificate requirement, and approval notice for returning from leave of absence.

Advisor Meeting and Approval

In principle, you must meet with your student advisor and obtain the advisor's seal on your application to return from leave. The sealed application is used to obtain permission to return.

Required Documents

Submit the required documents to the Student Welfare Desk. If your leave of absence was due to illness or injury, attach a medical certificate.

Notice of Permission

After the formal approval to return has been granted, a notice of permission to return will be mailed to you.
