Replacing a Lost Student ID Card

Start the reissue procedure promptly

If you lose your student ID card, promptly complete the reissue procedure at the Student Support Center (Student Welfare).

Submit the reissue application form

For students in the 2025 academic year in the Faculty of Future Science at Tokyo Senju Campus, fill in the required information on the printed reissue application form and submit it to the Student Support Center (Student Welfare).
