Where to Submit Leave, Return, and Withdrawal Applications

For students covered by the stated campus, faculty, and academic-year scopes, the Student Support Center’s Student Welfare Services desk handles leave-of-absence, return-from-leave, and withdrawal procedures.

Student Welfare Services desk

The Student Support Center’s Student Welfare Services desk, located on the third floor of Building 2, handles applications for a leave of absence, return from leave, and withdrawal. It also handles other student-status procedures, address changes, extracurricular activities, scholarships, student ID cards, certificate and student-discount pass issuance, student insurance procedures, student-life consultations, lost and found items, and assistance for students’ off-campus activities.
