Documents Required After Missing a Final Exam

For the 2026 academic year at the Saitama Hatoyama Campus Faculty of Science and Technology, the required document depends on why you could not take the final exam.

Transport disruption or illness

If you missed a final exam because of a transport suspension or delay, you need a delay certificate proving the disruption occurred on the commuting route shown on the back of your student ID card. If you missed the exam because of illness, you need a doctor's certificate stating that the exam date fell within your treatment period. The treatment period must be shown either as a date range or as the approximate number of days from a specified date.

Death of a relative

If you missed a final exam because of the death of a relative or a similar bereavement, you need a funeral attendance notice or a comparable document.
